Patients to consider for CBT-I

Worth an assessment when an adult has:

  • Trouble getting to sleep, or long waking periods, three nights a week or more for three months or longer.
  • Insomnia that persists while depression or anxiety is already being treated.
  • No improvement from general sleep advice, or a patient who would rather not add another sleep medication.

Adding CBT-I to a current therapy plan

Nobody has to change therapists. CBT-I is a short course that runs beside the work your patient is already doing.

The clinician reads the daily sleep diary, sets a sleep window from it, and moves that window week by week.

What about sleep apnea?

The two often sit together, and each is treated on its own. We handle the insomnia and refer out for apnea assessment when the picture calls for it.

Try the score yourself

Answer all seven questions about the past two weeks. The Insomnia Severity Index totals them into a score from 0 to 28. Nothing is sent anywhere, and nothing reaches a record at Oregon Sleep Center.

For the first three questions, please rate the current (last 2 weeks) severity of your insomnia problem(s):

1. Difficulty falling asleep
2. Difficulty staying asleep
3. Problem waking up too early
4. How satisfied/dissatisfied are you with your current sleep pattern?
5. How noticeable to others do you think your sleep problem is in terms of impairing the quality of your life?
6. How worried/distressed are you about your current sleep problem?
7. To what extent do you consider your sleep problem to interfere with your daily functioning (e.g., daytime fatigue, mood, ability to function at work or with daily chores, concentration, memory) currently?

A score is not a diagnosis. Take the result and the sleep history to a licensed clinician. No particular score is needed to refer.

What the Oregon program involves

Patients can book without a referral. Book an appointment.

We reach the patient

We call, arrange the assessment, and check what their insurance covers.

Treatment begins

The first appointment is 50 minutes on sleep history, previous treatment and medication. Then weekly visits, with a daily sleep diary in between. Most courses run six to eight weeks, and the sleep schedule changes as the diary does.

You get updates

With your patient's consent: Insomnia Severity Index scores, and the milestones as treatment moves.

Appointments are by video with clinicians licensed in Oregon. We do not see patients in person. The patient needs to be in Oregon for the whole appointment.
